After the completion of the form below, one of our qualified support staff will contact you to confirm your appointment.

PLEASE NOTE THE FOLLOWING: Submitting the request does not guarantee the appointment date or time. Before the appointment is set, one of the Performance Orthopedics staff will contact you for final confirmation. Do not show up to your appointment request without first confirming with a staff member. This is an online appointment request form and service. It may take up to one business day to process your request. If you need an urgent appointment, please call us directly at 248-988-8085 during regular business hours for faster service. If you have a true medical emergency, please call 911 or go to the nearest emergency room for treatment. The appointment requests will NOT be checked over the weekends or holidays. The on-call physician cannot make appointment requests over the weekend or after hours.
