* Only with proper authorization/referral for treatment. If your insurance plan requires a referral from your primary care physician you must have this before treatment is given. Appointments without this will be re-scheduled. Please make sure that all proper information is present to avoid disappointment or delays in care

** Worker’s compensation patients need proper authorization for treatment. This means an authorization number or name with whom we can verify coverage. Claims that are disputed or under litigation must have a State of Michigan Notice of Dispute form on file before any surgery will be performed or treatment is continued.
